Thanks for the feedback on the feedback.;-)

Yes, fields can be left blank.  I'm certainly open to suggestions for 
changes or improvements, though.

Right now, the messages are just mailed to me.  I'll use a mail filter to 
drop them into a separate mailbox.  Further processing will depend on the 
volume.  At the most basic level, I already have AppleScripts and MacPerl 
programs which could turn them into web pages.  I'd be surprised if we 
got enough volume to justify a spreadsheet or database, and statistical 
analysis.  Such would certainly be possible.  I deliberately formatted 
the messages consistently so I could extract the data easily.
