2013 Young Dancers Workshop Student Forms
Once you have received your acceptance notification (via email) you must download and fill out the following forms to complete your registration process.
Please mail these forms along with your final balance payment due May 27.
*Your registration will not be complete without receipt this final package.
NOTE: A copy (both sides) of your current health insurance card must be included to complete this packet.
